What is Form I-9?
According to the United States Citizenship and Immigration Services (USCIS), Form I-9 is used for Employment Eligibility Verification prior to beginning work in the United States.

Why do I need to fill out Form I-9?
According to USCIS, all employers must comply with US immigration laws which require they:
- Verify the identity and employment authorization of each person they hire;
- Complete and retain a Form I-9, Employment Eligibility Verification, for each employee.

Who must fill out Form I-9?
According to USCIS, a new Form I-9 must be completed each time an employee is hired to perform “labor or services in the United States in return for wages or other remuneration”.

I completed a Form I-9 when I started employment with UW-Madison, but I left my position. I have returned to UW-Madison, what do I need to do with the Form I-9 now that I returned to UW-Madison?
According to USCIS, when an employee is rehired after a break in service within three years from the date their previous I-9 was completed, a Supplement B rehire can be performed instead of creating a new Form I-9. Employees must provide a document to verify their identity, and in some cases, up-to-date work authorization as well.
